Submission of Membership Form & Fees, as shown in the table given below
MEMBERSHIP FEES ( one time ): Payment by Demand draft payable at Chandigarh , in favour of ‘ Academy of Prosthetics and Orthotics should be sent with the Membership Application Form to: The President, APO, 1134 Sector 44-B, Chandigarh-60047 (India)
ONLINE SUBMISSION OF MEMBERSHIP FORM WITH FEES: The Membership Application Form may be copied & pasted in a document file, duly filled and then mailed as an attachment to drvjsvohra@medireh.com , also intimating that fee has been paid online.
Your one time fee can be paid online, by clicking the Paypal button in the appropriate column in the table below, and by doing so you truthfully commit that you are eligible for the membership you have applied for. Your case would be duly processed and within 2 weeks your certificate would be despatched to you. If it is found at any later stage that any information regarding your qualifications or experience is false, it would lead to immediate cancellation of your membership.
However, it would be appreciated if the necessary copies of your certificates, duly attested, may be sent by post or courier at the above mentioned address.
